Living Annuitant Switch Forms
If you want to switch the portfolio/s in which your existing investments are invested complete Form 1 below.
And, if you want to switch the portfolio/s from which your drawdown amount is disinvested complete Form 2 below.
(This form is in respect of the portfolio from which your pension is paid.)

The HR158 Form is only used in conjunction with the HR157 UCTRF Retirement Form.
